Converting Rear Brakes From Drum To Disk On 97' DX

My apologies if this has been covered. I'm new to this forum and still trying to figure out how to navigate efficiently.

What parts are needed to convert my 97' DX rear drum brakes to disk brakes?

^^^ Thanks! There's lots of info about conversions, but none of them list the actual parts needed. That's exactly what I needed!
