Five bolt hub retro/switch

Hey there guys (and gals) I've got an '05 ex 2dr and i'm interested in changing my four bolt hubs to five bolt ones. I'm wondering if anyone has done it and if so what parts need to switch over. I'm pretty sure i'll have to switch the rear to disc. which will be cool all in itself. Please help need info!!!!!!
