I've heard and seen it before. You're not the first. Call them.

So this has nothing to do with the factory base mount and bush?
the defect is in the coilovers itself?

I figured that it would be easy as buying a new base mount, bush and reinstalling it.....
crap...

Same thing happened to me when i had Tein ss.
I couldn't figure out the problem.
I reused all of stock upper mounts. That might be why.
Just buy new upper mounts and reinstall them..

I think my rear left is coming out too. My car is lower on the rear left and now I know why. Is your car lower on the side that the bushing popped out? Are you going to notify tein about this? This might be a defect that Tein and the other members should know about. Can you post what you did to fix this?
Thanks for posting this problem.

This is what TEIn wrote back to me.
But I'm pretty sure we used everything hen the coilovers wher installed....

Sam,

Can you please take a look at the last page of the instruction manual for the 2001 Honda Civic Super Street kit. Here is the link ( http://www.tein.com/ti/inst/ka22u.pdf ). Can you please check if you have the collar installed between the washer and bushing then base mount. If you do not have the collar installed please install it because if you do not have it installed it will create a problem. If you do not understand what we are talking about can you please call the number that is listed below. Thank you.

Best regards,
