Replacing lock cylinder

I'm having a problem replacing the drivers' side lock cylinder on my 2008 DX. Part #72185-SNA-A01. Using this video for instructions:
Am stuck around 14:52, placing the part in position. I can get it installed, and it looks perfectly in place on the inside, but the lock cylinder is not perfectly lined up on the outside. If it's not perfectly lined up on the outside, the key won't turn in the lock, and if you try forcing it in anyway, it gets stuck. See pics:

How can this be wrong?

As you can see the lock cylinder is turned a little counterclockwise rather than being straight vertically, and it's slightly too far to the right and down. No matter how many times I try to shift it while reinstalling it, it always ends up like that.
So what am I doing wrong on the inside? How do I adjust it on the inside so that it lines up perfectly on the outside. I've unscrewed it and reinstalled it about a dozen times, but it always goes back in the same position. That's how it fits.
