engine stumbles when cold

engine stumbles when cold
its not my civic but its my mom's 2000 camry v6 with 75k miles. The engine would stumble every 5 seconds or so when it is cold and with the auto trans in reverse backing out of the garage, and the engine doesnt seem to rev as easy as before once warmed up. I am thinking it might be the fuel filter but i want to get you guys' input first. The fuel filter has never been changed. Thanks.
