Lets say, I am driving a and have to put it in the second gear while its running on 7-10 miles an hour, ( this is actually not a load but pushing car to run in the second gear) thats where i hear it. Or in other word, if its running on 37-40 MPH and its in its 5th gear and try to push the gas to increase the speed. that's where i hear the noise coming from the engine or somewhere, cant fingure this out. I hear this in the old cars when they have lots of mileage.

I dont think so. But if my guess is right of what you are trying to say, then its not the case that because I am changing the gear in the low RPM, if you get an idea of what i m tring to say, you might hear this type of noise in the older cars, when you try to push the car to get in the speed, this type of sound comes out.

It's just the car struggling. When I have alot of people driving around, the engine sounds awful. Takes a day of sitting before it goes away too.

Well yeah. My car has the same miles and does it. I've always noticed it since the car was new almost. Maybe throw in some fuel system cleaner (greased lightning). That made most of the noise go away during normal driving. When the car is loaded down, or AC on, the engine struggles.
