How-To: Fitting atop engine blew out and now leaks gas... HELP
I put the correct amount of seafoam into my gas tank early yesterday morning and when I got home last night about midnight, my car started chugging and reeking of gasoline. I popped the hood and saw a jet stream of gasoline spewing from this fitting.. what do I do?
birds eye view of engine east of air filter atop/east of air filter fitting that is spewing gasoline

Did you have the intake manifold off recently? You're missing a nut (red arrow) and your fuel rail is not aligned/seated on at least that fuel injector (red circle).

I'd actually be willing to bet those snapped off (at least the plastic where those bolts screw into) while either tightening or loosening. That plastic got brittle over time, at least in the ones I've seen.
In the coolant system's defense, at least it's only the overflow, and not the actual radiator cap missing.
Serious question, do you know who worked on this car last? Well, did anything major on it that required removal of the fuel rail, IM, and valve cover?
