Car jerking when cruising at all speeds

Unhappy Car jerking when cruising at all speeds

I am at a total loss with my car. Lately it feels like I fix one thing and another issue pops up. At first the car was jerking when accelerating and decelerating, the problem is still persistent. Now the car idles at 1500 rpm's at a cold startup, at any given speed the car shakes but most noticeable in 1st-3rd gear.

What can the issue possibly be?

Car Info:
2003 Honda Civic LX (5-speed)
146k Miles

Things Replaced:

Struts & shocks all around
NGK Spark plugs (last year)
Front engine mount
Transmission mount
Passenger engine mount
Valve cover gasket (still has a very small leak)
Cam plug

I am hoping that the issue is the rear engine mount, haven't had time to replace it.
Also I know for sure that the input shaft bearing went out as you can hear the bearing spin in 1st and 2nd gear.

Have you tried a smoke test for vacuum leak(s) ?
