The difference is that projectors (when designed right) spread light evenly and allow a sharp cut off on low-beam. This prevents stray light from rising too high and blinding oncoming drivers in traffic. Most modern day headlights use a parabollic multi-reflector that is designed to reflect light accurately and get the same effect of even lighting and sharp cut-off, like on the Civic.
Projectors probably cost more and need to be designed with proper optics. The problem with mutli-reflector designs is that although they prevent a lot of stray light, there is still some glare. That is why most HID don't use multi-reflector, but use projector or that "unclear" lens (don't know what it is called) like on the Mercedes-Benz cars or Acura TL for HID.
