Trouble accel from stop

My car:

97 sedan
automatic
142k

Here's my problem:

After coming to a stop at either a red light or stop sign, when I go to accelerate, my car begins to shake and doesn't accelerate properly. After rolling forward and shaking for a few seconds the car accelerates suddenly and off I go. I haven't had a problem with the first stop, only after I drive for a while, and it only seems to happen 30 to 40 percent of the times I stop at a light. Also, it is worse going up hills. It seems to help if I accelerate as slowly as possible, but Im not sure. Also, it seems as though the car is shifting roughly at other times when I'm driving, but maybe that is my overactive imagination.

I changed the transmission fluid this past summer, and checked the level and it's fine. Also, it has not been leaking. I also checked the CV boots and they appear fine, one is slightly warped but its not torn or leaking.

I am wondering if anyone has any advice or knows what my problem is or suggests that I stop driving the car if the problem will keep getting worse. I will probably end up taking the car into a transmission specialist either monday or next weekend. Thanks for any help!
