95 civic lx fault code 5. what does this mean?

95 civic lx fault code 5. what does this mean?
After replacing the clutch the check engine light came on and my radiator fan is not working so the car wants to overheat (during hot weather) after it reaches operating temp. I checked for the code and it appears to flash in groups of 5 (4 long 1 short). Anyone know what that code means? Fan relay? heat sensor? dead fan? Any info would be very helpful. Thanks
