CLutch chatter?

CLutch chatter? Just a few days ago I noticed my car shudders when I shift from 1st to 2nd gear under moderate acceleration. It does it after the engine is fully warmed up and I have driven for a few miles. Now the car shakes when I start out in 1st gear after it is warmed up. I think the clutch is chattering during engagement. There is no noise but it is not a smooth take-off when I start out. Could there be a problem? I have a 2004 Civic LX 5 speed with 21,000 miles on it. Please respond.
