Question Popping out of 2nd gear.....

whenever i leave my house and start driving, my car always seems to have problems goin into 2nd gear...and when its time to go into third, its shoots the shifter out of its place and causes a minor grinding noise....is this due to lack of letting the car warm-up, or is it a defect in the car???lemme know if im the only one with this problem
