Looks like a combination of brake fade and driver error.....
I say Driver error because he should have noticed that the brakes were starting to go earlier on the course, so he would have known to start brakeing earlier. Brake fade isnt instantanious ya know. other wise, if some thing broke.... Then there wasnt any thing that could have been done.

That was the Instructor.

its allways a good idea to turn off the ingnition after a crash, as it reduces the chance of a fire.
