Code 1 O2 Sensor / D15b Vtec

Recently purchased 88 Wagovan. Dropped in D15b Vtec. Had issued passing enissions. Found Code 1 which was O2 sensor, replaced O2 sensor and passed emissions. On way home from inspection and engine through code 1 again. WTF is going on? This is driving me crazy.think the engine is running RICH. What else causes the engine to run rich. Bad injector's? Bad TPS.

Please help.

Take off your heat shield and look for cracks on the exhaust manifold. Could be a vacuum leak too.
