Question Sudden Clutch Noise and Failure

My son and I replaced his 2k EX clutch, ThrowOut bearing, pressure plate and rear main a few months back. We used an Exedy kit.

Everything has been great for the past 2 months until today. My son said he was driving and when he tried to shift into 4th, it was hard, and made a clunk sound. The clutch pedal went to the floor and wouldn't come back up.

He turned the car off, and did an inspection under the hood, which showed nothing unusual. He then pulled up the clutch pedal by hand and it stayed up. After a few pumps of the pedal, it seemed to be fine.
My son drove the car home but when he is letting the clutch out, he hears a clicking sound.

I seem to remember reading on one of these forums, that someone had a couple of the pressure plate fingers break off and get stuck in the clutch basket, and it made a similar sound. If anyone out there has experienced this kind of problem, please let me know what you found as the problem and what you did to fix it.
