There was one washer I was unable to fit. The one that is located where the cable is connected to the shifter assemble itself, not the the shift rod. Where does the washer go and how do I fit it on? Also now I have a slight rattling sound whenever I shift to second and fourth.

The washer from the original shifter assembly should fit the Revo Technica short shifter. If it doesn't fit, then you might just want to head over to Home Depot and pick up a washer that will fit.

The rattling sound when shifting to second or fourth comes from the loose metal clip that secures the right shift cable to the short shifter. I just wrapped duct tape tightly around that.
