Lug stud problem?

Lug stud problem?
I just got new rims and tires for my 02 EX from Tire Rack. They are 15x7 with 195/60/15 tires on them. When installing them, one of the lug nuts from the original wheels would not come off easily. It was extremely tight the entire way off. It felt to me like I was turning the wrong end of it at certain points (like I was loosening the back end of the stud instead of the lug nut, not sure if that is possible or not). I noticed that one of the threads on the stud near the wheel end of the lug nut was missing. Putting the new lug nut on was difficult, though not as bad as the removal. I also noticed again that one of the other studs on that same wheel looks slightly crooked, though not enough for me to be completely sure it wasn't just an optical illusion. I remember having thought the same thing in the past, not sure if it was the same wheel or not though. For about a week, everything seemed fine with the new wheels, but then I started hearing a noise that I recognize from when I had a problem with loose lug nuts. It is a rapid thumping noise that occurs when coasting or "almost coasting". If the gas is pressed in enough, or the brake is pressed hard enough, the noise stops, but if I'm in neutral (5 speed) or barely braking or barely pressing the gas, the noise comes back. The first time I heard it, it got progressively worse, and luckily I got it to a shop in time, they said they found all of my lug nuts were loose, except the wheel locks, on every wheel, though its possible only one wheel was loose enough to cause an issue. Tightening the lug nuts seemed to be a permanent fix for the issue. This time it sounds more like it did in the early stages, and it I have checked the lug nuts and they aren't loose, except on one stud on the wheel that was a problem getting the lug off, probably the same lug, but without removing the nut I can't be sure, and I'm afraid if I remove it, I won't be able to get it back on. If I tighten the nut, it will stop the problem for a very short time (a mile or two), then it will start again. So far, every time I check the lug nuts, one is loose enough for me to be able to make a quarter turn or so on it to tighten it. I did this twice yesterday with about 15 miles driven in between. From what I have read though, one loose lug nut should not cause the noise I am hearing, and I am sure the noise is the same as what was previously diagnosed as loose lug nuts. Does anyone have any ideas?
