Possible dirty/bad injector?

Hey guys, so I'm in the process of elimination on why my car is running really lean. I even switched back to the stock map on my flashpro, and even at idle the rpms kinda bounce on me now and again, and my air fuel goes 14-17. And that's at idle. Part throttle it's ok, but 60% throttle plus it still climbs to the 17, even occasionally 18 range, and the engine has cut in and out on me in vtec when I'm on the gas. Needless to say I've stayed out of vtec and have been driving like an old ladie. But question is, since this even happens on the stock map, would it be my mods? 3.5in ktuned sri,header,test pipe, 3in. Catback. Or could a or a few clogged Injectors cause this?
i am also running a lean bank 1 code

Tried cleaning the intake sensors?

I really wanna get this ironed out cuz I dont want to take it to the tuner and waste their time if I have some other issue going on

if you have access to looking at the fuel trims, I'd look there first.
