RPMs going crazy?

RPMs going crazy?
I haven't been here in a while but I've got a strange issue and no one seems to understand me...
When I'm driving on the freeway, in either 4th or 5th gear, sometimes when I step on the gas I see the RPMs go up quickly - say from 3000 to 4000 for example - then drop back down to 3000 and climb slowly/normally back up to 4000 as I pick up speed. Someone mentioned to me that it might be a problem with the cluster not correctly showing RPMs but my issue is that I can feel the car doing this.

is the engine revving up while the guage is going up?
meaning, are the rpms of the enigine consistent with the guage reading?
if the answer to that question is yes, then it sounds like your clutch is slipping

Yes, I can hear the engine revving along with the RPM spike.
Sweet baby Jesus, thank you guys I was starting to really worry because it is doing it a bit more now. I've been told my engine is already on its way out because after about a year and a half (I bought my 01 used and salvaged at 80k and am now at 155k), it burns a quart of oil ever 500 miles. I know I worsened or quickened its death by not knowing this and running my engine for many miles without oil.
