Valve Guide/Seal Replacement

So my '01 4dr 5spd has been having oil spitting out the exhaust for the past 6+ months. It started when the valve clearance on cylinder#2 got way tight (like 3-4 full turns on the tappet) and made my engine run real rough and pop codes. After pressure testing I found there was something with #2 so I performed a valve adjustment and re-learned the idle. The engine ran like new, although the oil issues still remain. It is spitting oil out my exhaust (covers the rear bumper and exhaust tip). I have to add somewhere in the neighborhood of 1qt every oil change or so.

To my question: what do I need to do to replace the valve guide seals and possibly the guides? Do I need to completely yank the head or can this be done on the engine? I am pretty competent when it comes to mechanics, I have just never done valve/head work before and wondering if anyone has any insight to this problem. I assume either the jack-wad that did my timing belt either short-cutted the job; or with the valve getting that far to tight, something might have got damaged from ignition back-pressure

Short history of engine:
Has roughly 159,000 miles
Been mostly on top of oil changes, running 5w-20
Timing belt broke a little over a year ago, (had head supposedly milled/reworked as some valves broke)
