p0138 code ecu

Can anyone please help me...

my engine light is ON for almost 5 weeks till now...& ODBII reader shows code
P0138 O2 Sensor Circuit High Voltage (Bank 1 Sensor 2)

what is it about? (bank 1 sensor 2) ?
do i need to replace the primary(top) or secondary(bottom) sensor?
& how much for a sensor?

hey mon. bank 1 = first set of a two set sensor, bank 2 considered second set of two sensor set
bank 1 sensor 2 = first set of sensors or only set of sensors on our hondas. and the last sensor on the exhaust or furthest away from the engine. basically the sensor after the catalytic converter.

if you have a ex model, then you have 2 sensors, primary and secondary, but if you have an DX, lX, or HX, you have three sensors, primary, secondary, and third 02 sensor.

Now if you have an ex, then its the one after the cat. If its a dx, lx or hx, then its the second one on the exhaust manifold. or downpipe, whereever that second one is.
