The part you need can be found here https://www.hondaautomotiveparts.com...interior-light look at number 11 for the base. The parts come in the two main interior trim colors for about $35. You can also get the lenses for about $5 apiece. (numbers 7 and 9). The two bulbs are $4 apiece (number 3). I selected 2006 Civic Sedan EX automatic for my diagram choices. If yours is different (automatic/manual shouldn't matter for this part) choose from the menu.

The diagram shows the sunroof switch as a separate part, so you will likely have to move that center assembly over to the new one (unless that is the problem, then it is another $39 part.)
