Strut bolt keeps working loose

Replaced struts last week, and the p/s top mounting bolt keeps working itself loose. What did I forget to do?

Torque it?

Was the nut reused?
Was it supposed to have been replaced?

Some are only supposed to be one time use.

I have taken my front struts off many times and always re use the same bolts. Never had an issue with it not staying tight.

You could use a lock nut but you shouldn't have to, it should stay tight without one.

No, used the new bolts. Looking at it again, the bolt has a hex key attachment in the center. Does this need to be tightened?

Hex key in the center?

Which bolt is it? The crash bolts that attach to the knuckle or the ones on the strut mount? The only one that usually has a hex key in the center is the strut rod which does not get turned. That hex key is used to hold the rod steady while you tighten the nut.
