Uneven wear on brakepad?

I changed my brake pads yesterday and noticed that 1 of the 4 pads had uneven wear. One side of the pad had more wear than the other. This only occured with one pad. Anyone know why this happens?

The rotors were machined once a couple years ago. I doubt this has something to do w/ it?

Also, I replaced my factory pads w/ new oem honda ones. Why do the new ones come w/ more shims than factory? The new pads came w/ 3 shims for each side of the car. From the factory, the civic only came w/ 1 shim per side.
