Car Overheating Need Help Fast!!!!!!!!!!

I am new to this site and to civics but please help me out. I have a 94 civic dx sedan. My A/C never worked in my car. My problem is that my car is overheating. Now i have replaced the thermostat, the fan temp switch/sensor, the cooling fan relay, and i have flushed the old coolant out of the car twice and put new coolant in. When the car is in idle the fan kicks on when the car is getting to hot but for only like 15 seconds then goes off and when i am driving it does the same thing it only stays on for about 15 seconds and goes off. Why wont it stay on to cool the car down? I don't know what else to do and the water pump is working because the top radiator hose is hot and the bottom is cold and when it over heats bubbles up in the recovery tank. 15 seconds is the usual cycle. it doesnt stay on forever and usually the fan does very little of the cooling. the bottom hose should not be cold. it should be cooler than the top hose, but it should still get fairly warm, if not hot, depending on outside temps. if it really is cold, you have a clogged radiator and coolant is not circulating thru the engine properly.
